ABSTRACT

OBJECTIVE To investigate the effect of phosphotyrosine interaction domain containing 1 (PID1, NYGGF4) on promotion of IR and HCC, and explore its underlying mechanisms. METHODS Lentivirus were used to mediate the knockdown of PID1 in HFD induced IR mouse model as well as ob/ob mice. Intraperitoneal glucose and insulin tolerance were performed 4 weeks after lentivirus injection. Hydrodynamics-based transfection was applied to inducethe liver specific overexpression of PID1. Flow cytometry was exerted to detect the proportion and function of immune cells. qRT-PCR and Western blot were used to detect the expression of downstream pathways of PID1.Immunoprecipitation was used to determine the receptor of PID1. Chromatin immunoprecipitation (ChIP) was operated to measure the modification of H3K4me3 of PID1 promoter. RESULTS PID1 restriction improved insulin resistance, hyperglycemia and fatty liver. Conversely, hepatic knockdown of PID1 attenuated liver xenografted tumor growth. Moreover, PID1 liver- specific protooncogenes via hydrodynamics- based transfection established a primary hepatocellular carcinoma mouse model, induced an immunosuppressive environment, with the reduction of CD3 +, CD4 +, CD8 +T cells, retarded maturation of dendritic cells (DCs), pronounced differentiation of regulatory T cells (Tregs), and recruitment of MDSC. In addition, PID1 overexpression activated proliferation related genes, promoted anti- inflammatory genes, suppressed pro-inflammatory genes, induced glycolysis and lipid metabolism genes to facilitate tumorigenesis in liver. Importantly, PID1 exerted its tumor-promoting function through binding to epidermal growth factor receptor (EGFR) and activation of downstream MAPK pathway. As such, PID1 exist trimethylation of histone H3 at lysine 4 (H3K4me3) modification and IR up-regulated the expression of PID1 by activation the H3K4me3 modification. CONCLUSION PID1 is a new gene that exerts both liver cancer-promoting and insulin resistance inducing function. IR accelerates liver cancer development and progression partially dependent on the activation of PID1.

ABSTRACT

Objective To investigate the correlation between micro?bleeds and hemorrhagic transformation( HT ) after ischemic stroke. Methods One hundred and forty?five cases patients with acute ischemic stroke who were admitted to Luhe Hospital of Beijing Affiliated to Capital Medical University from 2009 to 2014 were retrospective analyzed. The MBs T2??weighted gradient?echo MRI was performed within 48 hours after admission to examine if there was a micro?bleeds,and the incidence of HT was assessed using follow?up brain MRI or CT in 2 weeks after admission. There were 125 patients with stroke were in without HT group and 20 patients were in HT group. Results There were no significant differences in terms of MBs ( P=0. 170 ) , gender( P=0. 671 ) , age ( P=0. 528 ) , hypertension ( P=0. 597 ) , diabetes ( P=0. 571 ) , hyperlipidaemia ( P=0. 772),smoking(P=0. 672),history of stroke(P=0. 469),coagulation function(P=0. 527),antiplatelet(P=0. 642),anticoagulation(P=0. 611) in HT group and without HT group. Atrial fibrillation((45%(9/20) vs. 22%( 28/125 ) , P = 0. 034 ) , NIHSS score ( 10. 9 ± 0. 5 ) points vs. ( 7. 8 ± 1. 2 ) points, P<0. 001 ) , thrombolysis(20% (4/20) vs.5% (6/125),P = 0.015) have significant differences in two groups.In multivariate logistic regression analysis,atrial fibrillation( OR=2. 421,95%CI 1. 823-3. 256,P=0. 012) ,NIHSS score( OR=9. 303, 95%CI 3. 094-27. 970, P<0. 001 ) were independent predictors for HT after ischemic stroke. Conclusion There is no relationship between MBs and HT after ischemic stroke. Atrial fibrillation,the severe of stroke are independent predictors for HT after ischemic stroke.

ABSTRACT

<p><b>BACKGROUND</b>The New Rural Cooperative Medical Scheme (NCMS) has been further adjusted and optimized to reduce the financial burden of rural residents and to achieve universal coverage for them. In this study, we aimed to explore the impact of NCMS on medical service utilization and medical expense of inpatients in recent years.</p><p><b>METHODS</b>The research data of Hainan Province were extracted from the Chinese NCMS platform from 2012 to 2014. Detailed information included total expenditure, average inpatients costs, average out-of-pocket payments, actual reimbursement rate, and average annual growth rate of the above indicators. Descriptive analysis was used to gauge the effects of NCMS.</p><p><b>RESULTS</b>In the utilization of medical services, NCMS inpatients in tertiary hospital decreased from 25.49% in 2012 to 20.39% in 2014, inpatients in county hospitals increased from 39.49% to 55.92%, simultaneously. The total expenditure in county hospitals rose steadily from 28.46% to 46.66%, meanwhile, the total expenditure in tertiary hospitals fell from 60.44% to 44.51%.The average out-of-pocket costs of rural inpatients remained stable over the years. Furthermore, the compensation fund of NCMS inpatients grew significantly. The actual inpatient reimbursement rate at township health centers increased from 76.93% to 84.04%. Meanwhile, the rate at county hospitals and tertiary hospitals increased slightly from 59.37% and 46.10% to 61.25% and 47.71%, respectively.</p><p><b>CONCLUSIONS</b>With the improvement of the reimbursement ability, especially after the new health care reform in 2009, the NCMS have been playing a prominent role in alleviating the economic burden of farmers' medical treatment. Meanwhile, more patients go to primary hospitals than tertiary hospitals, and the capability of primary hospitals has been greatly improved.</p>

After the concepts of electronic health records were described , the construction of service system for sharing electronic health records in some major foreign countries was analyzed in aspects of their organizations , poli-cies and laws, criteria for data sharing, data security and privacy protection, with suggestions put forward for the construction of service systems for sharing electronic health records in China, such as speeding up the transforma-tion of governmental management functions , strengthening the guidance for health information construction at macro level, establishing the criteria for data sharing, improving the legal system construction, and attaching importance to privacy protection .

ABSTRACT

<p><b>OBJECTIVE</b>To assess the effect of longterm and lower oral inoculation with Porphyromonas gingivalis (Pg) on the progression of atherosclerosis in apolipoprotein E-knocked out (ApoE(-/-)) mice.</p><p><b>METHODS</b>Six-week-old male ApoE(-/-) mice were inoculated orally with 0.1 ml live Pg(10(13)/L) or bouillon culture-medium quintic per week for 15 consecutive weeks, altogether 75 times of inoculations. The lesion area of atherosclerosis in the aortic tree was measured by en face quantification by red oil O staining method. The atherosclerotic lesion was examined by histopathology. The levels of total cholesterol and triglycerides were compared.</p><p><b>RESULTS</b>At 22 weeks after inoculation, the mean atherosclerotic lesion area in inoculated mice was (98 363.68 ± 12 043.00) µm(2), which was significantly greater than that in noninoculated mice, which was (62 985.06 ± 7419.64) µm(2) (P = 0.035).</p><p><b>CONCLUSIONS</b>Longterm lower oral inoculation of Pg can accelerate the progression of atherosclerosis in apolipoprotein E-knocked out mice.</p>

ABSTRACT

In order to analyze the molecular epidemiology of A (H1N1) influenza virus in 2009, the complete genome sequences of influenza strains from different host sources downloaded from the NCBI were analyzed on genetic evolution by DNAstar software in this research. The results showed that 79 mutation sites of new A (H1N1) influenza virus were observed compared to previous human A (H1N1) influenza strain, including 14 mutation sites new in all A (H1N1) influenza sources and 37 mutation sites only observed in swine strain. A significant difference was represented in antigenic sites between new A (H1N1) influenza strain and the previous human A (H1N1) strain. This phenomenon shows the new A (H1N1) influenza strain is either originated from the recombination of human and swine strain or from the infection in pig populations and gradual mutation to human tansmission, which remains to be further studied.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the prevention and the treatment of the complications of laparoscopic cholecystectomy, including bile duct injury, bile leakage and bleeding.</p><p><b>METHODS</b>1,100 patients undergoing laparoscopic cholecystectomy from October 1993 to January 2003 were retrospectively analyzed.</p><p><b>RESULTS</b>Operative complications included bile duct injury in one case (0.09%), bile leakage in three cases (0.27%), and bleeding in four cases (0.36%). All these complications were cured using operation and non-operation methods.</p><p><b>CONCLUSIONS</b>To prevent bile duct injury, bile leakage as well as bleeding, abnormal cystic duct and cystic artery should be identified, and electric damage to the surrounding tissues should be avoided during operation.</p>
